  • Please note that completion of this form does NOT guarantee placement in our program.
  • Please make sure to list 2 different references: 1 reference for your FAMILY from church staff or small group leader and a DIFFERENT reference for your STUDENT (teacher, coach, student sunday school teacher at church etc).
  • As part of your application, please submit the 4 Registration Documents.

The New Family Application Fee $100 is non-refundable. The Member Family Fee $450 will only be assessed if we are able to offer you placement into our program and is then nonrefundable at that point.
